A separate application for each team must be submitted by June 13th, 2008 along with the entry fee of $ 350.00. Confirmation of your acceptance will be mailed by June 30th. If not accepted, your check will be returned. Only $ 150.00 can be refunded if a team cancels after acceptance has been mailed.

REGISTRATION:
To be accepted for the tournament, your application must include:
-Completed registration form, including a team roster with player names, birthdays and all of the coach/team information.
-A check or money order payable to Port Chester Soccer Club for $350.00 and send it to
